Modification request form

This form should be completed if you wish to have a modification made to make your home more accessible. This can include grab rails or ramps.

Your Occupational Therapist, GP or support provider may be organising and paying for the work.

Disability modification – South Australia Housing Authority

If you are living in a community housing property and you need modifications to live independently, you may be eligible to have them installed through your community housing provider. Sometimes you may be offered another more suitable property rather than having a modification installed in your current home.

Alteration request form

This form should be completed if you want to make general changes to your home or garden. This can include erecting a carport or shed, painting walls or installing an air-conditioner.

Application forms

If you would prefer to complete the forms in person, visit our office or contact us to request a paper copy to be sent to you.

Once approved

If approved, you will receive confirmation in writing from Westside Housing.

General conditions include:

  • You pay for the cost of the work.
  • If plumbing or electrical work in being carried out, Westside needs a certificate of compliance from a licenced tradesperson when the job is complete.
  • If you leave the property you will either leave the item behind or return the property to its original state.
  • Westside will not pay you for items you leave at the home if you vacate.

In some cases you may need to meet some conditions, which include:

  • Council approval for items such as car ports, sheds, front verandas or patios.
  • Other conditions vary depending on the type of work being done.
